Event Request Form

Thank you for considering New Life for your event needs. We will do our best to help you be successful in your upcoming event. In order to support all community, ministry and outside events connected with New Life, every event should adhere to the process below:

  • Fill out Event Request form 6-8 weeks prior to event (for best results). Forms can be submitted online or to the church office. Do not schedule or promote your event until you have received approval from the New Life Staff Coordinator.
  • New Life will review your event considering our mission and other determining factors, such as other calendar events.
  • The New Life Staff Coordinator will contact you 7-10 days from submittal with approval status and any questions or other details we may need.
  • Once approved, you may be contacted with opportunities for marketing and promotion as determined by New Life's mission.
  • A new request form will need to be completed annually for ongoing meetings or events.

Please note - The sound and computer systems require trained personnel to operate. An additional fee may be assessed.

In requesting use of the facility I take full responsibility to make sure that the room is cleaned up and all tables, chairs, equipment etc. are returned to their original places after the event is over. If the kitchen is being used I also take responsibility in making sure that all trash is taken out and that the dishes are cleaned and put away. I will ensure that the thermostat is set at 78 degrees when I leave.
